There's a section of the Ethereal Plane here called the Curtain of Vaporous Color. That's where dreams are, apparently. And sometimes they end up more permanent than dreams. But nobody knows how, or at least the book I read didn't.
Good question. [The exact specifics aren't there, which is a little frustrating.] The way it's worded, seems like they mean it becomes part of reality.
